  3. PERSONAL APPEARANCE CONTRACT 
  4.  
  5. TERESA T. TALENT, referred to as TALENT, and SALLY S. SPONSOR,
  6. referred to as SPONSOR,  agree: 
  7.  
  8. TALENT shall appear on SEPTEMBER 11, 1993 from 10 AM to 2 PM at a
  9. A remote broadcast for  SPONSOR.  
  10.  
  11. TALENT shall provide the following equipment:  
  12.  
  13. SEE ATTACHED LIST, EXHIBIT 1 
  14. At least two working days prior to the appearance the SPONSOR
  15. shall insure that adequate support facilities are provided for the
  16. equipment as follows: 
  17. SEE ATTACHED LIST, EXHIBIT 2 
  18. SPONSOR shall complete and return to TALENT the attached "Remote
  19. Checklist form" two business days prior to the scheduled event. 
  20.  
  21. SPONSOR is responsible to provide the agreed upon support 
  22. facilities or personnel. In the event that the support facilities 
  23. agreed upon are not provided, the TALENT may cancel or abridge 
  24. the appearance; however, the full agreed upon fee shall be paid. 
  25.  
  26. SPONSOR shall provide the following equipment and support: 
  27.  
  28. SEE ATTACHED LIST, EXHIBIT 3 
  29.  
  30. SPONSOR shall at least two working days prior to the appearance
  31. verify that all equipment and personnel support stated are 
  32. prepared and shall submit a "Remote Checklist form" two business
  33. days prior to the scheduled event. 
  34. SPONSOR is responsible to provide the agreed upon equipment. In
  35. the event that the agreed upon equipment and support are not
  36. provided, the TALENT may cancel or abridge the appearance;
  37. however, the full agreed upon fee shall be paid. 
  38.  
  39. The SPONSOR shall pay to talent a total fee of $ 1234.56 ( one
  40. thousand two hundred thirty four & 56/100  dollars) for the
  41. appearance. This fee shall be due, in full, unless the event is
  42. canceled a minimum of FORTY-EIGHT hours prior to its scheduled
  43. start. The appearance may be extended provided that the TALENT
  44. agrees. An additional fee of $ 123.45 ( one hundred twenty three &
  45. 45/100 dollars) per half-hour shall be due. The TALENT may
  46. condition the extension of the appearance on prepayment.

  49. Food and beverage shall be complimentary to TALENT during the
  50. event. 
  51.  
  52. The SPONSOR shall be entitled to use the name and image of the
  53. TALENT in connection with the appearance.

  57. This is the entire agreement between the parties, and there are no
  58. other agreements. This contract may only be modified in writing.
